Family Medicine opening in IN Located in Crown Point Chicago 45m Full-time; Permanent Seeking a Board Eligible/Board Certified Family Medicine physician to meet the growing demand for primary care services located in Crown Point, Indiana. Family Medicine specialists see patients of all ages, from birth to seniors. Our providers diagnose and treat medical illnesses, provide preventive care such as annual checkups and health risk assessments, and provide guidance on maintaining a healthy lifestyle. In addition, they manage chronic conditions and can refer to specialists or coordinate hospital care as needed. WHAT WE NEED: Candidate would thrive in a fast-paced environment and be team oriented. This physician will need to work collaboratively with colleagues and Advanced Practice Providers to coordinate and deliver the highest quality of patient care. Candidate will also have a desire to develop strong ties with patients and the community in which they serve. They must display a professional skillset and be willing to build long-lasting relationships with patients and their families. WHAT YOU CAN EXPECT: Schedule: 36 hours patient contact and 4 hours of admin work Flexible schedule includes one evening per week until 6:30 p.m. and one Saturday per month from 8:00 a.m. until 12:00 p.m. Practice Hours: Monday Friday varies by day, Saturday 8:00 a.m. 12:00 p.m. Average outpatient volume: 15 18 patients per day Telehealth offered Onsite lab and onsite x-ray Strong mentorship from existing providers WHAT WE OFFER: We strive to create a flexible and realistic work-life balance for our providers.
