Great opportunity in the Northern Orange County area of CA for a Nurse Practitioner to perform annual wellness exams working for a private group.  Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) 2+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus Perform an annual wellness exam for senior Medicare patients Collaborate with Primary Care team to ensure completion of holistic patient care Combination of home health and telehealth - 30 mile radius  If you are not located in the Northern Orange County area you must be comfortable commuting to the Northern Orange County area and then an additional 30+ miles within the Northern Orange County area Must have active CA license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have AWV exp.
