this full-time (5 days per week) primary care role, you will have the opportunity to manage complex
patient cases in post-acute settings (skilled nursing and independent living facilities) in Media,
Pennsylvania, focusing on improving care for some of the most vulnerable patients. You will work
alongside a collaborative interdisciplinary team and lead initiatives that make a tangible
difference in patient health and quality of life.Job DutiesPrimary care rounding in skilled nursing
facilities Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of
careCollaborate with clinicians, facility leaders and others regarding care and servicesBuild
relationships with your patients, patients' family members, and facility staff to facilitate
effective treatment deliveryProvide oversight, guidance, and mentorship to our nurse
practitionersConduct clinical case reviews, as needed and share tools, information, and guidelines
as they relate to quality of careRepresent TeamHealth on appropriate external levels engaging and
establishing/maintaining relationships within the post-acute care communityCollaborate with
leadership to review care coordination initiatives, develop collaborative intervention plans, and
share ideas about network management issuesQualificationsMinimum of 1 year of experience, with a
commitment to delivering high-quality, patient-centered carePost-acute care experience is
beneficial, though not required, as we provide comprehensive support and development
opportunitiesActive and unrestricted Pennsylvania license (or in process)Active and unrestricted
DEA/prescriptive authority (or in process)A deep commitment to improving patient outcomes, leading
clinical excellence, and driving change in the healthcare landscapeInternal medicine (IM), family
medicine (FM), emergency medicine (EM), hospitalist or candidates with any related experience are
welcome to applyCompensationCompensation is fee for service (FFS). Expected compensation is an
estimated range of $180,000 to $291,000 annually with no cap on productivity income potential.Ready
