Job Requirements: Must have a current, valid, unrestricted Physician license in the state of New York throughout employment. Must attain and maintain active staff clinical privileges appropriate to duties and responsibilities. Must hold and maintain current, valid and unrestricted federal and state registrations to prescribe and dispense controlled substances. Current Board Certification in Family Medicine. If not currently Board certified, candidate must be Board eligible. Graduate of an accredited residency program.
