Geriatric/Palliative Care Physician in Houston, TX

Geriatric/Palliative Care physician The Geriatric/Palliative Care physician will follow the patient as part of their panel and will be expected to manage the patient to a conservative pathway. This will involve addressing daily issues via phone, as well as in-person visits. The physician will also be expected to prescribe meds to address daily issues. Our Physicians are quality driven. They apply their clinical expertise in a managed care, population health context. They serve patients across the country through a national telehealth platform. They promote palliative care where clinically appropriate, patient education, autonomy, and quality of life issues. This job is ideal for physicians who are forward-looking and embrace value-based care. Qualified physicians must be board certified in palliative care / geriatrics or both, must be currently licensed in the state. This is a full-time role, 5 days a week, Monday – Friday.
