Post Acute Care Nurse Practitioner

We are searching for a part time Post Acute Nurse Practitioner in Providence, RI. This position involves delivering comprehensive medical care, managing chronic and acute conditions, and optimizing the well-being of patients as they transition from acute care to post-acute care.
