Your responsibilities include:Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per dayCollaborating with interdisciplinary teams to optimize patient outcomesEnsuring smooth transitions of care with hospice, palliative care, and home care agenciesMaintaining accurate and timely documentation in our EMR system (Gehrimed)Qualifications:Graduate degree as a nurse practitioner or physician assistantActive and unrestricted NP or PA license in IndianaStrong clinical assessment and diagnostic skillsExcellent communication and interpersonal abilitiesExperience in post-acute care preferred, but not required (training available)Our compensation is a productivity-based model (Fee for Service).
