Job Description
Are you a new grad or experienced post-acute care nurse practitioner (NP) or physician assistant
(PA) who aims to make a difference? TeamHealth needs someone like you! We are seeking an NP or PA to
work in a skilled nursing facility rounding 2-3 days/week. This opportunity includes the flexibility
to set your own schedule. We have no start/stop times so you can round in the morning or in the
afternoon and still have great work-life balance.Key Responsibilities:Perform primary care in a
skilled nursing facility setting for short term and long-term patientsManage chronic disease,
diagnose illnesses and medical conditions, and implement plan of careOrder and interpret lab
resultsPrescribe and monitor medicationsConsult with physicians and coordinate patient care with
other healthcare providersEducate patients and their caregivers on coping skills to handle chronic
and age-related conditionsBuild relationships with your patients, patients' family members, and
facility staff to facilitate effective treatment deliveryPromote healthy lifestyle changesDesired

your patients.Our compensation is a productivity-based model (Fee for Service) that offers you
control to maximize your income potential. Expected compensation is estimated salary range of
$45,000 to $65,000 annually with no cap on productivity income potential. This is a part-time 2-3
