Baylor Scott & White Hillcrest – Cardiac Rehab Department -Provides Cardiac and Pulmonary Rehab as a physician referred Phase II service line. Registered Nurses provide evaluations for patients as they enter the cardiac and pulmonary programs, establish their plans of care, provide lifestyle and behavior modification, develop exercise prescriptions, provide high quality cardiac and pulmonary care education, and review telemetry while under direct observation during exercise. In addition, the RN will see Phase I inpatients prior to hospital discharge to provide education to ensure outpatient care is achieved.
