This full-time position involves providing clinical and outpatient services for high-risk older adults in subacute and long-term skilled nursing facilities. The successful candidate will be part of a primary care team dedicated to holistic care, including acute, chronic, preventive, and palliative care services. Responsibilities include comprehensive evaluations, diagnostic tests, prescribing treatments, and patient counseling. Location: Austin, MN, 40 miles from Rochester, MN. This role involves travel within the Southeast Minnesota region. Key Aspects: Collaborative work with advanced practice providers. Balance of subacute and long-term care patients. Medical directorship opportunities. Blend of primary outpatient internal medicine or family medicine, and senior services care delivery.
