Ref #: M-RN-294748 We're looking for a compassionate, skilled RN to join a home health team in St. Albans, VT on a contract/travel basis starting ASAP. This role averages a 37-hour week with a pay rate of $2,501. What you’ll bring Current RN license in Vermont. 1–2 years of nursing experience (home health experience is a plus). Strong clinical assessment and critical-thinking abilities. Clear communication skills and the ability to work independently while managing multiple patient visits. Valid driver’s license and reliable transportation. BLS certification required; other certifications a bonus. What you’ll do Perform thorough patient assessments, create individualized care plans, and evaluate outcomes. Deliver nursing care including medication administration, wound care, and chronic disease management. Teach patients and families about disease management, medications, lifestyle changes, and self-care techniques. Monitor and document patient progress, report changes to physicians, and coordinate with the interdisciplinary team. Follow infection control, safety, and regulatory standards. Provide emotional support and guidance to patients and families about home care resources. Take part in quality improvement efforts and ongoing professional development.
