Home IV Clinical Pharmacist
Job Description
Provides infusion therapy for patients in an outpatient setting. Provides clinical oversight and monitoring of therapies. Collaborate with nurses, providers and patients/caregivers. Makes members/patients and their needs a primary focus of ones actions; develops and sustains productive member/patient relationships. Maintains knowledge of and assures departmental compliance with Kaiser Permanentes Principles of Responsibility and policies and procedures, and applicable regulatory requirements and accreditation standards.
Essential Responsibilities:
- This position, knows and complies with all Kaiser Permanente quality, safety, and emergency policies and procedures. Demonstrates quality and effectiveness in work habits and clinical practice in every interaction with patients, colleagues, providers, and leadership. Ensures patient safety in the preparation and provisioning of care related to but not limited to medications, procedures, infection prevention, fall prevention, including consistent use of two patient identifiers and procedural time outs. Reports safety hazards, accidents and incidents, and unsafe working conditions promptly.
- Supervise, oversee and verify the preparation, packaging and distribution of sterile products and supplies by technicians and support staff.
- Review physician orders, provides drug information and provide pharmacokinetic dosing advice and customize therapies to the abilities and limitations of patients/caregivers.
- Troubleshoot problems with infusion medication therapies and administration devices.
- Order, monitor labs and adjust therapy based upon results and protocols and document clinical interventions.
- Counsel patients on drug therapy, potential adverse reactions, administration and use of infusion devices.
- Prepare, package and distribute sterile products and supplies.
- Train employees and students.
Basic Qualifications:
Experience
- Minimum two (2) years of experience as a pharmacist, including at least one (1) year of experience as an infusion pharmacist.
Education
- BS in pharmacy or PharmD.
License, Certification, Registration
- Pharmacist License (Colorado)
- National Provider Identifier required at hire
Additional Requirements:
- N/A
Preferred Qualifications:
- Home infusion experience.
Notes:
- Will need 1 year IV admixture experience.
- Will be covering all 4 infusion sites
