If so, TeamHealth has the right opportunity for you!Responsibilities: Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care in a skilled nursing facilityImplement an individualized plan of care for patients' goals and provide clear communication with patients, clinicians, facility leaders and staff regarding care and services for enrolleesWork alongside pharmacists on medication managementComplete acute, follow up, discharge, and wellness visits as necessaryMake recommendations on transitional care, DME, home health services, or outpatient services after short stayTimely response in chart completions and pharmacy recommendationsProvide oversite to TeamHealth clinicians providing coverage in the same facilityExperience as a medical director in a skilled nursing facility preferred.
