Clinic is seeking a Family Practice/Primary Care Physician and a Medical Director- details of each are below Family Practice/Primary Care Physician and NP needed in WI- can easily license.   This is open for Locums, Locums trial to perm and perm Duties/Responsibilities: ·     Provide primary care services including diagnosis, treatment and management of acute and chronic conditions. · Perform routine examinations, preventative care and health education to patients of all ages. · Develop and implement individualized care plans in collaboration with patients and their families. ·     Coordinate with multidisciplinary team members including specialists, nurses and social workers to ensure continuity of care. ·     Board certification in Family Medicine, Internal Medicine or related specialty PREFERRED. ·     Experience working in a primary care setting.   Medical Director Role – Locums trial to perm or direct perm – able to license   Job Summary: The Medical Director will oversee the day-to-day clinical operations of the Clinic. The Medical Director is responsible for planning and directing the clinic's medical policies and procedures. Additionally, the Medical Director will practice as a Clinic Physician responsible for providing high­ quality primary care services by diagnosing patient conditions using but not limited to, analyzing reports, test results, medical records, and immediate-family histories and including examinations to diagnose condition of patients and propose treatment options.   Qualifications: • Must hold MD or DO degree from an accredited medical school. •    Five (5) years of experience working with a community clinic is highly desirable. •    At least (5) years within a leadership and/or executive role.
