Summary
This position serves as an Advanced Medical Support Assistant (AMSA) to a Patient Aligned Care Team (PACT) in the Home Based Primary Care (HBPC) program.
Location is Batavia, OH with possible other listed locations available.
This position will require regular travel. DRIVER'S LICENSE REQUIREMENT: A driver's license in the State or Territory in which assigned is required. Candidates must pass a Federal service driving test and/or complete any related requirements.

Duties
VA Healthcare System Serving Ohio, Indiana, and Michigan (VISN 10) advocates for a Whole Health System of care in each of the Medical Centers. Major Duties:
Responsible for organizing and maintaining the complex day to day administrative operations of the HBPC and HBTC programs
Promotes a whole health approach into daily routines & practices by attending & applying whole health trainings/education
Encourages internal & external customers to incorporate whole health into daily routines, duties, responsibilities, and interactions
Performs work involving the collection, compilation and/or tracking of data and statistical information in support of the HBPC and HBTC program operations
Assists in the planning, review, and reporting of data/statistical results of the programs
Establishes and organizes computerized data sets and retrieves computerized data
Performs a basic analysis of the data and generates a variety of reports
Identifies initiates and organizes process for data collection for the program
Reviews documents and reports and ensures data entry is complete and accurate
Enters pertinent information into an automated tracking system
Maintains automated system of program-specific data to track and works with nurse manager and program specialist to implement a plan of action based on the data
Schedules and verifies scheduled appointments via scheduling package and via telephone calls
Responsible for ensuring scheduling is accurate by confirming appointments and verifying appointment completion with an entered note in the electronic medical record
Will triage inquiries from patients, caregivers, other agencies, and VA staff concerning the HBPC and HBTC programs & communicate as appropriate and per process and policy
Performs other related duties as assigned

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Must be proficient in the English language.
Experience/Education:
Experience. No experience required.
*Education. High school diploma, General Education Development equivalency certificate, or proficiency certificate from a State or territorial-level Board or Department of Education.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
To qualify for the GS-06 level, you must have one year of experience equivalent to the GS-5 grade level. Experience at the GS-5 includes but is not limited to: scheduling, cancelling, re-scheduling patients appointments and/or monitoring both inpatient and outpatient appointments for areas of responsibility, ensuring encounter forms are completed in order to obtain appropriate workload credit, verifying and updating demographics and insurance information, processing all emergency and non-emergency transfers to other VA facilities or private hospitals, performing basic eligibility, co-pays and preauthorization requirements for specific coverage, etc.
Candidates must also demonstrate all of the knowledge, skills, and abilities below:
Advanced knowledge of the technical health care process (including, but not limited to, scheduling across interdisciplinary coordinated care delivery and/or care in the community models and patient health care portals) as it relates to access to care.
Advanced knowledge of policies and procedures associated with interdisciplinary coordinated care delivery and/or care in the community operational activities that affect patient flow, and patient support care, administrative functions to include, but not limited to appointment cycles, outside patient referrals, follow-up care, overbooking, provider availability, etc.
Advanced knowledge of medical terminology due to the technical nature of language utilized by clinicians.
Ability to collaborate and communicate with a wide range of medical clinicians across multiple disciplines (e.g. medical doctors, nurse practitioners, physician assistants, psychologists, psychiatrists, social workers, clinical pharmacists, and nursing staff) to accomplish team goal setting to ensure medical care to patients is met.
Ability to independently set priorities and organize work to meet deadlines, ensuring compliance with established processes, policies, and regulations.
Ability to communicate tactfully and effectively, electronically, by phone, in person, and in writing, with internal and external customers. This may include preparing reports in various formats and presenting data to various organizational levels, as well as resolving patient concerns.
Preferred Experience: Microsoft word experience, Excel experience, scheduling patients, strong communication skills, customer service skills

The full performance level of this vacancy is GS-6. The actual grade at which an applicant may be selected for this vacancy is GS-6.
Physical Requirements: The work is sedentary.
