Primary Care Physician | PACE Program

Join a mission-driven, value-based care team as a Primary Care Physician in our PACE (Program of All-Inclusive Care for the Elderly) center located in Stockton, CA. We are seeking full time physicians who are passionate about delivering holistic, patient-centered care to older adults through an integrated care model that supports clinical autonomy and meaningful relationships with patients. As a Primary Care Physician in our PACE program, you will provide comprehensive primary care to frail seniors, collaborating with an interdisciplinary team to ensure optimal health outcomes. This role includes clinical leadership and offers a unique opportunity to make a lasting impact in a supportive, team-based environment.
