Summary
The Licensed Practical Nurse (LPN) provides safe, individualized nursing care in an ambulatory care setting at the Fayetteville Health Care Center (Raeford Road) location. Under the supervision of a Registered Nurse or provider, the LPN administers medications, monitors patient conditions, assists with procedures, provides patient education, and supports care coordination to ensure quality patient outcomes.

Duties
Duties include, but are not limited to:
Provides safe, individualized nursing care to patients in an ambulatory/specialty care setting under the supervision of a Registered Nurse or licensed provider.
Performs assigned patient care duties including observation, documentation, and reporting of patient's conditions, recognizing urgent or emergent situations and taking appropriate action.
Administers medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) in accordance with facility policies and documents all actions appropriately.
Assists with diagnostic tests, treatments, and specialized procedures, ensuring proper preparation, support, and follow-up care.
Provides patient and family education related to disease processes, medications, treatments, and care plans; reinforces instructions provided by healthcare providers.
Establishes and maintains effective working relationships with patients, families, and interdisciplinary team members to promote communication and continuity of care.
Maintains patient privacy, dignity, and confidentiality in accordance with VA policies and customer service standards.
Participates in performance improvement initiatives and contributes to maintaining a safe, clean, and therapeutic environment for patients and staff.
Performs support duties for laboratory services, including specimen collection, processing, and documentation, as applicable.
Serves as a preceptor and assists in training and orienting less experienced staff, including LPNs, Nursing Assistants, and Health Technicians.
Adheres to infection control, safety, and information security policies, ensuring compliance with all regulatory and VA requirements.
Work Schedule: Monday to Friday 08:00am-4:30pm
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education or Training. Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the NLNAC.
Exceptions include:
Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions.
Individuals granted a license by a jurisdiction that does not require graduation from an approved school.
English Language Proficiency. Licensed Practical Nurses must be proficient in spoken and written English in accordance with VA Handbook 5005, part II, chapter 3, section A, paragraph 3j.
Licensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
Grade Determinations. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.
GS-6:
Requires completion of at least one 1 year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN. In addition the following criteria must be met:
Technically proficient in initiating, performing & completing assigned duties in providing care to variable patient populations.
Knowledge & ability to appropriately carry out assigned patient care based on the patient' conditions; to use judgment in selecting the appropriate order & sequence of procedures & treatments; & to accurately recognize, report & record relevant patient information. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) for appropriateness & conformity with established policies/procedures.
Ability to observe, identify, & respond to the patient's needs for care, including medication, equipment-assisted care, & patient/family education.
Prepare & administer prescribed medication (oral, topical, subcutaneous, intramuscular, and/or intravenous) & performs treatments according to established policies/procedures. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly & accurately documenting noted changes, & reporting any deviations from normal to RN or MD/DO.
Knowledge & ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, & initiate appropriate emergency interventions as directed.
Knowledge & understanding of human behavior, patient motivations & reactions to situations, & ability to appropriately utilize this knowledge in working effectively with patients, family members, & other staff.
Establish constructive relationships with individual patients & their families to elicit feelings & attitudes, & to promote positive relationships, communication, & socialization skills. Fosters an environment of respect for individual patient & family rights to privacy & dignity in all aspects of care delivery. Effectively incorporates knowledge & understanding of established customer service standards in all interactions with patients, family members, &, or other internal/external customers.
Knowledge & skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing & handling specialized instruments or other specialized equipment, & monitoring the patient's condition before, during, & following the procedure. Serves as a preceptor in orienting, educating, & training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures.
Actively seek out educational opportunities to enhance nursing knowledge & skills, sharing new knowledge gained with other staff to improve & advance nursing practice.

Preferred Experience: Graduation from an NLNAC-accredited program preferred, but not required. 2 years ambulatory, primary care, or long-term care experience preferred.

The full performance level of this vacancy is GS-6.
Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, kneeling, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures.

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
