About the Position: This position is located within the Director of Mental Health, Social Work Services at Eisenhower Army Medical Center, Fort Gordon, Georgia. The primary purpose of the position is to provide follow up care supporting Mental Health Providers in a clinical setting on a highly specialized multidisciplinary treatment team dealing with mental health issues."

Duties
Provide follow up dealing with the identified high-risk population, supporting Behavioral Health Providers with commanders providing the best level of treatment available.
Ensure through facilitation, negotiation, procurement, advocacy, communication, empowerment, quality care, and involvement necessary for the achievement of mutually developed treatment goals.
Maintain ongoing comprehensive supportive care with identified high-risk soldiers during treatment.
Bring to the attention of the appropriate provider patterns or problems, at the client level or system level to promote smooth transition of soldiers through the treatment program.
Maintain ongoing follow-up with patients identified in caseload.
Perform direct patient support, to include assisting clinical providers to access appropriate patient referrals and resources and ongoing education of soldier, appropriate team members.

Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
This position has specific initial background investigation and periodic re-verification requirements since its duties require interaction with children and youth under the age of eighteen (18) years of age on a recurring basis.
Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS.
This position has a TDY or business travel requirement of 5% of the time to various locations.
The work will require the incumbent to work evenings/nights; rotating shifts, extended hours or other schedules common to hospital operations.
Incumbent may be required to perform call duties; work overtime to include weekends and federal holidays, out of cycle, or unscheduled training or on an emergency basis.
This position is designated as Inclement Weather Essential. The incumbent must make every attempt to report for work on time and/or remain on duty during severe weather conditions.
A pre-placement medical examination is required.
Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position.
Qualifications

In order to qualify, you must meet the education and/or experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Your resume must clearly describe your relevant experience; if qualifying based on education, your transcripts will be required as part of your application. Specialized Experience: One year of specialized experience which includes: (1) processing referrals of mental health patients; (2) providing resources or referrals to clients; and (3) maintaining case records. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-07).
OR
Education: One and a half years of graduate level education in a field directly related to the work of the position, such as Psychology, Human Services, Child & Family Welfare, Counseling (ex. crisis, substance abuse, mental health, chemical dependence), Family and Community Services, Behavioral Science, Human Development, Social Work, Sociology, Vocational Rehabilitation Counseling.

Combination of Education and Experience: A combination of education and experience may be used to qualify for this position as long as the computed percentage of the requirements is at least 100%. To compute the percentage of the requirements, divide your total months of experience by 12. Then divide the total number of completed graduate semester hours (or equivalent) beyond the first year (total semester hours minus 18) by 9. Add the two percentages.

FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university. Male applicants born after December 31, 1959 must complete a Pre-Employment Certification Statement for Selective Service Registration.
You will be required to provide proof of U.S. Citizenship.
One year trial/probationary period may be required.
Direct Deposit of Pay is required.
Selection is subject to restrictions resulting from Department of Defense referral system for displaced employees.

Salary includes applicable locality pay or Local Market Supplement.
