Summary This position is located within the Social Work Service at the Columbia VA Health Care System in Columbia, South Carolina, under the Department of Housing and Urban Development/Veterans Affairs Supportive Housing (HUD/VASH) Program. Learn more about this agency Duties ***THIS IS NOT A VIRTUAL POSITION, YOU MUST LIVE WITHIN OR BE WILLING TO RELOCATE WITHIN A COMMUTABLE DISTANCE OF THE DUTY LOCATION*** Major Duties: The Social Work Associate will provide a full range of direct care services in accordance with his/her limited Scope of Practice as outlined in the position description. This includes developing psychosocial assessments of clients in order to identify emotional, social, physical, familial, economic and vocational needs and resources. Identifies appropriate VA and community resources and facilitates utilization of these resources by the Veteran and families. Provides ongoing supportive counseling to patients to maximize their coping and adjustment to illness, injury, treatment, living demands and emotional crisis to help work toward goals of independence. s the veteran to complete applications for financial assistance, employment and housing. Identifies the target population and assist with program admission process and locating placement and assisting veterans to maintain placement providing follow-up to ensure continuity of care. Works with outpatients in a variety of settings such as the office, home, homeless shelters, etc. and special attention is given to the target population, chronically homeless veterans, that meet program criteria. Assists the veteran with the sustainment of their placement via direct services, supportive counseling, case management and treatment planning. Provides individualized services requiring the need for information, advice, referral services, etc. that assist a veteran out of homelessness. Performs other related duties as assigned. Work Schedule: Monday - Friday, 7:30am - 4:00pm or 8:00am - 4:30pm. Work schedule subject to change based on agency needs. Selective Service Registration is required for males born after 12/31/1959 Physical Requirements: The work is primarily sedentary and non-strenuous and is carried out within the confines of the office/workspace provided in the community or in Veterans Affairs Medical Centers (VAMCs) and Community-Based Outpatient Clinics (CBOCs). Some walking, climbing stairs, standing and handling and carrying light objects is required. No special physical demands are required to perform the work. A significant amount of walking throughout the Medical Center and in the community is required for face-to-face contact with patients, family members, medical center staff, and community agency staff. Qualifications For GS-7: Possess at least one year of specialized experience equivalent to at least the next lower level in the normal line of progression. Specialized experience is experience in a program of direct social welfare services that required the application of program policy and operations knowledge in providing social welfare services and/or assisting social workers. NOTE: Experience must be fully documented on your resume and must include job title, duties, month and year start/end dates AND hours worked per week. OR Possess one-full academic year of graduate study in social sciences. This education must have been obtained in an accredited college or university. OR Possess an equivalent combination of graduate level education and specialized experience as described above to meet total qualification requirements for GS-7.
