Nurse Practitioner – Mobile Program
Job Description
We
are seeking a Nurse Practitioner to join our Mobile Program in Bronx, NY,
working collaboratively
with PACE Physicians and community primary care
physicians to ensure the proper coordination of care
for participants alongside
the Interdisciplinary Team. The Nurse Practitioner will address
participants'
functional, clinical, and psychosocial needs, promoting independence and
overall
wellness.In
this role, you will serve as the liaison between participants, community
physicians,
nursing homes, and plan staff to develop and implement personalized
care plans. You will also be
responsible for developing person-centered goals,
communicating changes in participant conditions,
and facilitating primary care
preventive services.Key
Responsibilities:Collaborate
with PACE and
community primary care physicians to coordinate participant careAddress
functional, clinical, and
psychosocial needs with an emphasis on independence
and wellnessServe
as a liaison between
participants, physicians, nursing homes, and plan staffDevelop
and implement care plans, including
person-centered goal developmentCommunicate
changes in participant conditions to relevant team
membersFacilitate
primary care preventive servicesSchedule: Full-time 40 hours per week, 8am –
5pmCompensation
& Benefits: $180,000
- $190,000 plus full benefits packageRequirements:Licensed
to
practice as a Nurse Practitioner in NY.DEA
CertificationAANP
or ANCC Board CertificationBe
medically
cleared for communicable diseases and have all immunizations
up-to-date before engaging in direct
participant contact.Must
have a Medicaid Provider ID and a Medicare Provider ID in good
standing.Minimum
of one (1) year of experience working with a frail or elderly population, or
the
ability to receive appropriate training from the PACE organizationMinimum
of two (2+) years of
clinical nursing practice in home care, care management,
or a related fieldEffective
oral and
written communication and interpersonal skillsHigh-level
analytical and research skillsAbility
to
manage multiple projects and meet deadlinesBasic
computer skills, including proficiency in Microsoft
Office
