Put the passion back into your practice and gain control and autonomy over when and where you practice.Opportunity Overview:Uncapped productivity income potential with baseAverage 6-8 hours daily roundingLeadership stipendResponsibilities:Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care in a skilled nursing facilityProvide clear communication with patients, clinicians, facility leaders and staff regarding care and services for enrolleesWork alongside pharmacists on medication managementComplete acute, follow up, advanced care planning, discharge, and wellness visits as necessaryImplement an individualized plan of care for patients' goalsMake recommendations on transitional care, DME, home health services, or outpatient services after short stayTimely response in chart completions and pharmacy recommendationsExperience as a physician in a skilled nursing facility preferred. However, physicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), internal (IM), emergency (EM), primary, adult, post-acute, home health, hospice, and palliative medicine.Our compensation is a productivity-based model (Fee for Service).
