Job Description

Primary location for position is located in Fernandina, FL - Nassau County Part Time: 24-32 hours a week Job Description APRN - Home Palliative Works in collaboration with a physician and other palliative care team members in an active practice to deliver palliative care and related health care services to patients and families in a variety of settings including home palliative care. Job Description Responsibilities: • Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation and education for both patients and family. • Assists with diagnosis, treatment and management of acute and chronic health conditions. • Orders, performs and interprets laboratory and radiology tests within scope of professional practice. • Writes orders pertaining to informed consents, diagnostic procedures and treatments. • Prescribes medications and other treatments. • Administers medications as prescribed. • Consults with palliative care physician or designee as needed, informs attending physician of services provided and collaborates with other physicians as needed. • Facilitates clarification of patient and family goals of care. • Prepares and maintains accurate patient records, charts and documents to support sound medical practice and reimbursement of services provided. • Executes physician’s orders in accordance with legal, ethical, and organizational standards. • Complies with applicable laws and regulations. • Initiates reimbursement for services rendered on a timely basis. • Ensures provision of informed consent for CPR and DNR. • Recommends medications as required to treat specific populations. • Adheres to the practice of confidentiality regarding patients, families, staff and the organization. • Provides educational sessions to advance palliative medicine among internal staff, medical community and community as a whole. • Participates in on-call coverage and assists in establishing schedule. • Participates in regular interdisciplinary team meetings to review active cases. Qualifications: • Graduate, advanced education with a Master’s degree • Minimum of two (2) years of nursing experience. • Experience in hospice palliative care preferred • Valid State of Florida Driver’s license with driving record acceptable to Community Hospice insurance carrier. • Advanced Practice Nurse or PA license in the State of Florida Job Type: Part-time Expected hours: 24 – 32 per week Benefits: • Paid time off Work Location: In person
