Role Description
The Patient Safety Specialist is a key contributor to the Patient Safety Function within the Clinical Operations and Learning Team. The role champions the organization's mission to promote a culture of safety by supporting Patient Safety initiatives and the organization's Safety Committee. This role will work closely with leaders and contributors across Clinical Operations and Learning and Oak Street Health Centers. Success requires a high ability to collaborate, problem solve, manage projects, work independently and be flexible.
Core responsibilities
  • Assist in the administering Monthly Safety Committee Meetings
  • Review, monitor, and classify safety events. Provide central follow-up to close out events
  • Investigate systems issues and participate in sanctioned safety analyses
  • Track and trend findings on routine walkthroughs conducted by the care teams and incidents reported by team members
  • Collaborate on the development of policy and procedures affecting organizational safety
  • Diagnose existing workflow gaps that affect patient safety and recommend solutions
  • Assist in the creation of process mapping, standard operating procedures, and other efforts as needed
  • Coordinate the dissemination of safety-related education and communication to field leaders and teams on the resources and processes that support safe care and environments
  • Support patient safety projects needed
  • Other duties, as assigned

This role reports to the Director of Patient Safety.
What are we looking for?
We're looking for a motivated Patient Safety Specialist with:
  • 2+ years of experience in healthcare setting
  • Bachelor's degree strongly preferred
  • Ability to travel- 60%
  • Hands on experience assisting or facilitating local implementation and/or process improvement projects
  • Strong critical thinking and proactive problem-solving skills
  • Outstanding written and verbal communication
  • Ability to manage several projects and processes, staying organized and on-time with timelines
  • Ability to collaborate and build relationships with a wide range of partners to achieve shared goals
  • Solid group facilitation and presentation skills
  • Ability to strive in a fast-paced environment
  • US work authorization
