Job Description
Overview
The Community Health Worker reports to the Director of Outreach, CINQ Care Inc. The Community Health Worker is responsible for primary work in the community with specific target populationโs in order to educate, enroll, and manage health home members. Community Health Workers work closely to meet and exceed our mission to provide a collaborative and comprehensive approach to healthcare, training, and research.ย The Community Health Workers embody CINQโs core values, including;ย Caring, Expertise, Dedication, Imagination, Partnership, Forthrightness, Humility, Courage, and Relentlessness.
Primary Responsibilities
ยท The Community Health Worker will have the following responsibilities:
ยท Convey the purposes and services of a program to the user population and the impact that program or service would have
ยท Assist in recruitment and retention of GBUAHN HH member
ยท Assist care team to help patients develop health management plans and goals
ยท Assist care team in coaching patients on problem-solving, effective management of their chronic health conditions and self-care
ยท Assist patient in understanding care plans and instructions
ยท Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place
ยท Liaison at sponsored community outreach functions
ยท Work collaboratively and effectively within a team
ยท Establish positive, supportive relationships with participants and provide feedback
ยท Help clients in utilizing resources, including scheduling appointments, and assisting with the completion of applications for programs for which they may be eligible
ยท Assist clients in accessing health-related services, including but not limited to obtaining a medical home, providing instruction on the appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services
ยท Assist CINQ members in conducting community strength and needs assessments as part of the care coordination process
ยท Assist clients in translation and interpretation where appropriate
ยท Motivate patients to be active and engaged in self-management and promote treatment adherence
ยท Advocating and promoting health and wellness, utilizing health volt as a tool for implementation
ยท Advocate for harm reduction
ยท Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program.
ยท Identify and apply appropriate role definition and skilled boundaries

The Community Health Worker shall have the following duties:

ยท Represents CINQ and its subcontractors by displaying a respectful and caring manner with members and their families.
ยท Addresses all member concerns in a timely and efficient manner and reports any complaints to the Care Coordinator for resolution.
ย Compliance:
ยท Complies with quality assurance, OSHA, HIPAA, patient-centered medical home focus, infection control, safety, and other policies set forth.
ยท Maintains competence through continuing education and in-service training
ยท Attendance and being on time, and ready to work your scheduled hours is an essential job requirement.

Qualifications
The Community Health Worker should have the following qualifications:
Fluent in Spanish and English
Education:
- Associate degree required; B.S or B.A. preferred.
- Prefer 3-yearsโ experience in a health and social services environment or education
Experience:
- Working background in health and social services preferred.
ยท Verifiable good driving record and reliable transportation
ยท Bilingual/bicultural (Spanish) encouraged to apply
Technology:
- Computer literacy, including proficiency in Microsoft Word and use of online reference and information resources required.
