WestCoast Children’s Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. Position Overview

The C-Change Case Manager works in collaboration with WestCoast clinicians and parents, social workers, probation officers, teachers, and other adults in youths’ lives to provide advocacy and linkage to support clients' mental functioning. These services may include coordinating with education and vocational  programs, housing, primary health, psychiatry, benefits and other independent living skills services. The C-Change Case Manager must have strong advocacy skills, demonstrate flexibility, utilize a strengths-based approach, and have demonstrated experience working with high needs youth.

How the C-Change Case Manager makes an impact:

  • Provides advocacy, targeted case management, and community based rehabilitation services to transition-age youth.
  • Assists youth in identifying and utilizing community services and resources to address their needs, including: housing, medical/physical health, education, employment, independent living skills, advocacy, placement stability, and age-appropriate enrichment activities in Alameda and SF county.
  • Provide risk assessments, crisis intervention, stabilization, safety plans, and drop-in counseling (as needed).
  • Connects youth with legal representation, as needed and appropriate, and supports youth in court appearances.
  • Provides psycho-education about sexual exploitation and advocacy services for clients.
  • Assists youth with accessing SSI and/or regional center services, appropriate housing resources including acute and structured residential treatment options as appropriate.
  • Follow up with youth when incarcerated or hospitalized and transitioning to placements.
  • Assists youth with obtaining and keeping Medi-Cal.
  • Complies with policies, procedures, documentation and paperwork for EPSDT Medi-Cal billing and meets billing requirements as set by the agency.
  • Provides services in the community , for which travel by car is necessary.
  • Demonstrates professional and ethical conduct at all times as a representative of the WestCoast community.

The ideal candidate has…

  • At least 2+ years of experience working with systems involved youth.
  • Bachelor’s degree in Psychology, Criminal Justice, Social Work, or a related field preferred.  
  • Bilingual (Spanish) preferred.
  • Experience providing case management and rehabilitation services.
  • Demonstrated experience working with adolescents with dual diagnosis and high mental health needs and/or Sexually Exploited Minors.
  • Knowledge of Alameda and SF County community resources for TAY youth.
  • Experience working either in the community or in a variety of settings: homes, schools, neighborhoods, other agencies, etc.
  • Experience in working with diverse communities. 
  • Ability to work with clients who have complex traumatic histories and other attachment difficulties. 
  • Case management skill – assessment/evaluation, crisis intervention, ability to use supervision effectively, ethical and legal responsibilities, high quality documentation.
  • Cultural humility - knowledge of issues of race, class and ethnicity.
  • Efficiency and time management - ability to be well-organized.
  • Collaboration and team functioning - able to work independently and collaboratively as part of a multidisciplinary team.
  • Knowledge of or ability to learn Medi-Cal reimbursement procedures and billing.
  • Clean CA DMV record and current car insurance and car; travel is required.

Position details and compensation:

This is a full-time, hourly position reporting to the TAYS Program Director. Salary starting at $31.25 per hour ($65,000 per year), plus a 10% language differential if applicable. Includes competitive compensation and benefits package, 403(b) and incentive compensation plans.
