Care Navigator

We are looking for a compassionate and organized non-clinical healthcare professional who has a passion for helping people navigate obesity care at Found.   A care navigator is responsible for guiding and supporting members through the Found and traditional healthcare system. The primary responsibility is to provide personalized assistance to members, helping them navigate processes to ensure they receive the obesity care they need.  This responsibility can include ensuring complete medical history and medication information, barrier to care identification and resolution, and resource facilitation. 

Primary Responsibilities:

  1. Ensuring complete initial intake information, including medical history and medications, is collected prior to the initial provider consultation.
  2. Assisting members as needed in understanding their Found options and care plans.
  3. Providing support and encouragement to members throughout the medical weight care  process.
  4. Coordinating with providers to ensure members receive appropriate care, education, and follow up.
  5. Providing logistical support for members and/or providers.
  6. Facilitating communication between members, member support, and providers.
  7. Identifying and addressing barriers to care, such as financial/insurance or logistical challenges.
  8. Advocating for members’ rights and needs within the Found system.
  9. Maintaining accurate records of member interactions and issue resolution.
  10. Providing education and resources to members, as requested to help them better manage their health.
  11. Staying up-to-date on the resources available to members.
  12. Collaborating with other healthcare professionals, such as RN care managers, coaches, registered dietitians, and social workers to provide comprehensive obesity care.

Required Qualifications:

  • Excellent interpersonal and communication skills.  Navigators must have excellent communication skills, including the ability to explain complex medical or insurance information in plain language, listen actively, and build rapport with members
  • Ability to work compassionately and effectively with the public, as well as working with colleagues at all levels of the organization
  • Passionate about delivering safe, high-quality obesity healthcare, and harnessing technology to do so
  • Strong aptitude for problem solving.  Navigators must be able to identify and address barriers to care, such as financial or logistical challenges, and develop creative solutions to ensure members receive appropriate care.
  • Demonstrated ability to prioritize and manage multiple tasks and projects at various stages of completion, and ensuring their successful completion
  • Exceptional organizational abilities, with the ability to organize and prioritize competing priorities
  • Adept relationship development skills to work across departments with multiple different stakeholders.
  • Must be comfortable using technology, including electronic health records to document precise information such as medications, lab results, and other relevant health information.

Minimum required education and experience:

  • 1+ years of experience in a healthcare setting (i.e. clinic, medical office, pharmacy, health insurance company etc.)
  • 1+ years providing remote care or remote services (i.e. telehealth practice, customer service, etc.)
  • High school diploma or GED

Ideal education and experience:

  • 3+ years of experience in a healthcare setting (i.e. clinic, medical office, pharmacy, health insurance company, etc.)
  • 3+ years providing remote care or remote services (i.e. telehealth practice, customer service, etc.)
  • Experience working as a medical assistant, certified nursing assistant, pharmacy technician, or similar ancillary support role.
  • Licensure as a medical assistant, certified nursing assistant, pharmacy technician, social work, licensed practical nurse, or similar
  • Bachelors degree
