Case Manager
Job Description
This position is eligible for American Rescue Plan Act (ARPA) funds which includes a one-time $750 sign-on bonus payable after the completion of 90 days of employment as well as a monthly stipend of up to $694.50 which is based upon the hours worked in the previous month for as long as the funds are guaranteed by the State.
What You’ll be Doing
The Case Manager in our Residential Services Program will provide appropriate role modeling in a trauma-informed therapeutic environment that promotes the physical, mental, and behavioral health of each resident and assists them in developing life skills. The Residential Services Case Manager works as a part of a collaborative trauma informed therapeutic team, establishing open communications and support among co-workers, and provides case management services support and advocacy for adolescents.
Your Main Priorities
• Provides care coordination for all youth in the program related treatment team meetings, mental health, health, dental and all necessary appointments.
• Participates in youth interviews, intakes, and schedules treatment team meetings for each youth.
• Communicates with all key stakeholders regarding youth updates and coordination of care.
• Documents all communication with providers in EMR pertaining to youth status and interactions.
• Cultivates a trauma-informed environment by understanding the short and long-term impacts of trauma and neglect.
• Responds to residents and their family/caretakers with the consideration of empathy, sensitivity, respect, and maintain trauma-sensitive language and approaches.
• Collaborates as a member of the team of therapeutic staff in promotion of residential philosophy and program protocols.
• Ensure active engagement in day-to-day structured and unstructured recreational activities, psycho educational groups, regular resident house meetings, and life skill development and maintenance.
• Implements crisis intervention techniques in accordance with policy and procedures, and agency training; ensures non-violent physical interventions are used only as appropriate (i.e., client is immediate risk to self or others).
• Maintains positive and professional interactions with program partners and within the community (i.e., parents, police departments, DCYF, child advocates, etc.).
• Transports clients according to state and agency transportation policies.
• Other duties as assigned.
Qualifications
• Associate degree in social work, human services, or related field required; bachelor's degree in social work, human services, or related field preferred.
• Minimum of 6 months' experience in human services, preferably with children and/or adolescents.
• Experience working in a group home setting is preferred.
• Ability to communicate and effectively interact with a diverse population.
• Bilingual in Spanish strongly preferred.
• Strong organizational and time-management skills.
• Basic computer skills required; knowledge of Microsoft Office 365.
• Must have a valid driver’s license and auto insurance.
• Must be able to authorize Child & Family to complete Motor Vehicle Record check.
• The ideal candidate will be required to meet all pre-employment requirements such as background checks and medical clearances.
