Case Manager
Job Description
Case managers with the Hope, Unity and Freedom (H.U.F.) Center, you are provided the opportunity to make a difference in the daily functioning of adults in need. Case managers use psychosocial day support and interventions to assist adults in their care to meet the goals and outcomes designed by the individuals, the treatment teams, and you, the case manager. Case managers are the primary support system for those in their care while at the H.U.F. Center, improving the lives of those around them daily.
