Case Manager
Job Description
SUMMARY
This position is primarily responsible for managing program services for enrolled participants including the development of care plans, provision of linkages and referrals to care and follow-up activities, communication with medical/behavioral health providers and project staff to decrease barriers to access for medical care and support services. Assists in outreach and health education activities, as needed. Recruits individuals for, and conducts, evidence-based prevention interventions and counseling. Conducts data entry. This position reports to the Program Coordinator.
Essential Duties and Responsibilities:
Conduct GPRA Intake Assessments and Follow-up Assessments;
Utilize SBIRT model, including PHQ-9, CAGE, PCL-C, LEC, PSQ, and GAD-7 assessment tools;
Manage written and electronic participant records;
Participate in agency-sponsored and community outreach events, such as health fairs, LGBT events, etc.;
Provide presentations for internal and external programs to promote the program services;
Educate participants on substance use, HIV, and HCV prevention issues;
Recruit for, implement, and facilitate Seeking Safety EBP;
Assist with development and ongoing project implementation plan;
Participate in bi-weekly supervisory sessions with the Program Coordinator;
Complete documentation regarding participant assessments, consent forms, and demographic and contact information;
Conduct ongoing outreach & retention efforts via one-on-one contact and program events;
Re-engage participants for Follow-up Assessments at 6-months post-Intake Assessment;
Participate in b-weekly staff meetings, case conferences, and in-house and external trainings;
Ability to travel off-site to conduct outreach and attend necessary trainings and conferences;
Perform other duties as assigned.

Education and/or Experience:
Bachelorβs Degree and CASAC required; at least 3 years of experience in HIV/AIDs or behavioral health; Certified in HIV/Viral Hepatitis Testing and Pre/Post Testing Counseling required; bilingual in English/Spanish preferred.
High School Diploma or GED β CASAC-T with 4 yearsβ experience working with applicable populations who have substance abuse/co-occurring disorders.
Good verbal and written communications skills
Knowledge of HIV/AIDS, substance use & mental illness, and LGBT issues
Good problem-solving skills and ability to work autonomously
Must have computer skills (Microsoft Office, etc.)
Must successfully complete a criminal background check, obtain clearance from the Office of Children and Family Services and be drug free.
Employment Type: Full Time

Salary details: $40-43k annually.
