Currently, we are looking for the right person to join our Population Health team. In close collaboration with our care team and the physician, this Nurse Case Manager will telephonically and in clinic serve a subset of our patients who need care/case management services. This may include managing their transitions post-discharge from a hospital or other clinical facility, providing continuous clinical support to patients with multiple chronic conditions to ensure they have the resources needed to heal or sustain their health, and other opportunities when a physician identifies a patient who just needs a little extra support. To excel in this position our ideal new teammate will demonstrate professionalism through their reliability, communicate with kindness, hold an LVN or RN license, and have at least 2 years experience in a similar role.
