Case Manager Supervisor
Job Description
Wayside Recover Center is seeking a Full-time Case Manager Supervisor to provide support, supervision and oversight of the case management team and ensure team goals are achieved for the Family Treatment Center.
This role will also provide direct case management services to individuals accessing Wayside Services, including direct outreach, home visitation case management, advocacy services, resource linkage and recovery support. This position will also manage grant outcomes and compliance.
Duties including, but not limited to, the following:
- Provide weekly direct supervision, oversight, and support to a staff team of case managers.
- Recruit, train, hire, evaluate and manage disciplinary actions as necessary.Â
- Provide training and support to the team to ensure compliance with applicable ethical requirements, appropriate boundaries, and the ability to articulate the unique supervisory requirements of the case manager role.
- Facilitate case management team meetings, assist with training and education of case management staff.
- Provide trauma-informed and culturally intelligent care to a diverse population, with a focus on African American clients.
- Provide site specific and in-home case management for Wayside Recovery Center clients, including case management, education groups, individual sessions, resource linkage, collaboration, and coordination of care.
- Facilitate group services.
- Assist clients in setting goals based on their individual needs assessments, and in identifying steps to achieve their goals, monitor progress.
- Identify high-quality community providers and agencies whose services are relevant to client needs. Provide outreach services to engage and connect clients with the community agencies that will help meet their needs and goals.
- Coordinate scheduling and attend case staffing and appointments with community providers with whom client is involved. Maintain regular contact with client’s network of service providers.
- Make appropriate referrals and coordinate care to assist client in obtaining resources and meeting treatment planning goals.
- Provide required education for Statute 245G substance use disorder treatment program.
- Maintain accurate, timely and complete records, including but not limited to documenting progress, screenings, collateral contacts, and goal planning.
- Maintain on-going communication with multi-disciplinary team and collateral workers, including providing written reports and discharge information.
- Role model behavior that is consistent with and supportive of people in recovery from substance use disorder. Such behavior will include upholding ethical standards of Wayside Recovery Center programs, reliable attendance, mediation and/or conflict resolution, respectful assertiveness and open communication. Demonstrate respect and appreciation for diverse cultures and lifestyles.
- Collect and record data in a timely manner- as assigned for grant purposes.
Additional Duties:
- Assist in program planning, implementation, and evaluation.
- Participate in in-services, workshops and other continuing education relative to quality client care.
- Follow all Statute 245G and individual licensing requirements and standards, including, following confidentiality laws and mandated reporting.
- Provide transportation to client appointments and to obtain resources as needed.
- Provide crisis intervention/evaluation as needed.
- Participate in weekly supervision and staff meetings.
- Represent Wayside Recovery Center in the community in a positive and constructive manner.
QUALIFICATIONS:
Required
- Bachelor’s degree in behavioral sciences or related field
- Preferred: LADC or mental health licensure with license in good standing.
- Preference will be given to candidate with experience with mental health and trauma-informed care.
- Must be able to pass DHS background study and be free of communicable disease.
- 1 year supervision experience.
- Demonstrated experience working effectively with diverse individuals and communities particularly with populations including Black, Indigenous, and People of Color
- Demonstrated ability to work with and relate effectively to women and children of diverse economic and cultural backgrounds.
- Previous experience and/or understanding of substance use disorder and recovery.
- Skilled in the process of identifying and assessing a wide range of client needs.
- Knowledgeable about local community resources and how to use those resources for the benefit of the client.
- Must have current MN driver’s license and good driving record.
- Must have reliable transportation.
- No DWIs in last three years; No more than three moving violations/accidents in past three years
- Must be eighteen years or older.
- Strong attention to detail and flexibility; thinking capabilities and humor are welcome.
Preferred
- 2,000 hours of supervised experience working with individuals with substance use disorder preferred.
- Preferred: LADC or mental health licensure with license in good standing.
- Preference will be given to candidate with experience with mental health and trauma-informed care.
Physical Requirements:
- Must be able to lift 25 lbs.
- Must be able to climb stairs.
