Certified Nurse Practitioner - Geriatrics Hospice/Palliative

St. Josephs Health is seeking full time board certified Nurse Practitioners with interest and experience in Geriatrics, Hospice, Palliative Care, and/or home healthcare to join the Home Based Primary Care team in Onondaga County.

The Mobile Integrated Services Team (MIST) is dedicated to providing primary care in the home through house calls and management of patients in collaboration with their primary care providers or community providers. Our multidisciplinary team takes a holistic approach to care for medically and socially complex patients, enhancing quality of care and coordination while reducing costs for the most vulnerable populations.

OPPORTUNITY DETAILS:

• Provide multidisciplinary primary care and oversight to patients in home and community settings.
• Coordinate with healthcare providers and community services to optimize care.
• Offer behavioral care and social supports as part of primary care in the home.
• Respond rapidly to urgent and acute care needs, including palliative care and support for family members/caregivers.
• Conduct a mix of in-person and telemedicine visits, averaging approximately six visits per day.

REQUIREMENTS:

• Board Certified- Adult Gerontology or Family Nurse Practitioner Certification
• NYS License and DEA certification
• At least 2-3 years of Nurse Practitioner experience working with an elderly population is required.
• Experience working as a Nurse Practitioner in a Primary Care setting is preferred.

RECRUITMENT PACKAGE:

St. Josephs Health offers a comprehensive salary and compensation package that includes:

• Compensation: $105,000 - $125,000
