About the work:

The Client Care Coordinator is expected to perform a variety of duties that relate to client care including in-home care consultations with potential clients and family members, introductions for new clients and new Care Pros and reassessments with existing clients. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored care plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality of care, client satisfaction and retention as well as opportunities to increase service hours.

Responsibilities

  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a care plan
  • Conduct Service Inquiries and Care Consultations as needed following the consultative sales process
  • Maintain a scheduled nurturing plan with potential clients and families who have yet to commit to service
  • Document all prospective client activities in Salesforce
  • Document all referral partner activities in Salesforce
  • Work with the Honor Care Team to coordinate various aspects of a client’s care
  • Conduct client/Care Pro introductions with every new client and with every new Care Pro
  • Create and maintain client and responsible party records, documenting all reassessment meetings
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a reassessment program
  • Demonstrate open and effective communication with clients, family members and referral providers/care providers
  • Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team

About you:

To succeed in the role, you’ll need:

  • College degree preferred
  • One year experience in home care, health care or senior-related industry preferred
  • Must possess a valid driver’s license
  • Excellent oral and written communication skills and the ability to listen effectively
  • Ability to work independently, maintain confidentiality of information and meet deadlines
  • Ability to organize and prioritize daily, monthly, quarterly and yearly work
  • Ability to present a professional appearance and demeanor
  • Availability to work evenings or weekends as required

Our range reflects the hiring range for this position.  We use national average to determine pay as we are a remote first company.  Individual pay is based on a number of factors including qualifications, skills, experience, education, and training.

Base pay is just a part of our total rewards program.  Honor offers equity and 401K with up to a 4% match.  We provide medical, dental and vision coverage including zero cost plans for employees.  Short Term Disability, Long Term Disability and Life Insurance are fully employer paid with a voluntary additional Life Insurance option.  We offer a generous time off program, mental health benefits, wellness program, and discount program.

Hiring Salary Range
$45,000—$50,000 USD
Hiring Variable Compensation Range
$1—$5,000 USD
