Here's WhyClient Care ManagersLike Working for us: • Competitive Compensation • Medical, Dental, Vision Plans • Short Term and Long Term Disability Coverage • Generous Paid Time Off Plans • Bonuses • Paid training and professional development opportunities Responsibilities of the Client Care Manager: • Conducts client intakes, assessments, and regularly scheduled visits/wellness check-in calls • Develops and maintains the client care plan, identifies and reports opportunities to improve the client experience, and communicates with clients, families, care partners, and referral sources • Serves as a mentor to caregivers and supports their care delivery • Maintains complete, accurate, and timely client records • Generates regular streams of new business by building relationships with facilities and representing the organization at networking events • Maintains compliance with applicable laws and regulations and agency policies and procedures • Provides after-hours on-call rotation support for our clients, referral partners, and prospective clients - this support may include e-mail, telephone, and/or home visits Successful Candidates Possess: • The passion to surpass barriers, seize opportunities and go beyond the “call of duty” to improve the client experience • Minimum 2 years prior account or client management, preferably in the health/home care industry • Homecare sales and/or marketing experience is a plus • High school diploma or equivalent • Thrive in an independent, self-motivated environment • Great at building and maintaining relationships • A calm, positive demeanor at all times and is able to effectively multitask in a high-functioning office environment. • Excellent written and verbal communication skills • Be team oriented and comfortable collaborating with other staff • Tech savvy and proficient in Microsoft Office Job Type: Full-time Pay: $50,000.00 - $55,000.00 per year Benefits: • Dental insurance • Employee discount • Health insurance • Paid time off • Vision insurance Schedule: • 8 hour shift • Monday to Friday • On call Supplemental Pay: • Bonus opportunities Application Question(s): • Do you have a drivers licenses and your own reliable transportation?
