Clinical Pharmacist

Grace Care is seeking a passionate fully remote/telephonic Clinical Pharmacist to join our team! Candidates must be flexible, adaptable and above all, patient in all types of situations. The primary role will be completing Comprehensive Medication Reviews (CMR) with members. This position allows our elite clinicians to work from their fully remote home office. Full-time position Shift: Monday - Friday 8:00am - 5:00pm *Please note shift is subject to future change, depending on client needs.Candidates must have a quiet, distraction-free home office with high-speed internet. What You’ll Do: • Conduct telephonic or virtual Comprehensive Medication Reviews (CMRs) with patients. • Identify and resolve medication-related issues, ensuring safe and effective therapy. • Educate patients on proper medication use, adherence, and side effect management. • Document findings and recommendations in MTM platforms per program guidelines. • Communicate clinical recommendations to providers as needed. • Collaborate in an interprofessional environment to meet members needs! Requirements: • Active Registered Pharmacist license in the state of Pennsylvania. • STRONG computer and technical skills is a MUST (role will require navigation of multiple platforms) • Candidates must have a quiet, distraction-free home office with high-speed internet. • This is a full-time position with daylight shifts (exact shift depending on time zone) coverage of some holidays and weekends may be required. • Empathy, cultural competency, and a commitment to improving lives. • Reliable, proactive, and passionate Preferred Qualifications • 3+ years of experience in PBM/Medication Therapy Management experience • Retail, ambulatory care, or hospital pharmacy experience is taken into consideration with ability and intrinsic drive to take on new opportunities.
