Summary
Under the supervision of the Director of Pharmacy, the incumbent shall prepare, dispense and distribute medication and provide clinical monitoring and drug information in a manner, which ensure optimal patient care. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care.
Responsibilities
• Prepares medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities.
• Providing pharmaceutical care including anticoagulant, Pharmacokinetics, renal dosing, IV to PO, antimicrobial stewardship, and many other pharmacy protocols.
• Ensures that routine and priority tasks are completed within established departmental time frames.
• Prepares, administers, and documents medications and treatments in accordance with state and federal regulations.
• Contacts and confers with physicians concerning the appropriate drug, dose, dosing interval, drug allergy, and drug-drug interaction.
• Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
• Maintains pharmacy inventory to ensure that drugs are readily available for patient use.
• Mentors and fosters a constructive teaching environment that helps students, interns, and Pharmacy Technicians build confidence in their skills, knowledge and abilities.
• Assists DOP in developing and implementing new programs and policies and procedures.
• Actively participates in gathering accurate data for quality assurance issues, clinical pharmacy documentation, DUE, and ADR.
Requirements
• PharmD - Pharmacist - California State Board of Pharmacy
• Minimum 1 year experience in a hospital setting, Home Infusion, or Long Term Care required.
Job Type: Full-time
Pay: $68.26 - $72.42 per hour
