Job Summary: TheClinical Pharmacy Specialistunder the direction of the Pharmacy Director performs specified distributive and clinical pharmacy activities and related services as they pertain to providing safe and effective infusion therapies (in accordance with State, Federal and OSHA regulations) for the Jackson Park Hospital Pharmacy. Exemplifies the Jackson Park Hospital mission, vision and values and acts in accordance with Jackson Park Hospital policies and procedures. Essential Responsibilities: · Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems. · Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership. · Assure the appropriate selection, dosing, administering, and dispensing of medication orders in oncology. · Recommend formulary alternatives when appropriate. • Compound and dispense medications for accurate delivery to patients. • Provide drug information and education to medical staff and patients. • Communicate with physicians and other clinicians as necessary to facilitate care. • Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies. • Assist with academic functions such as serving as a preceptor for pharmacy students. • May float to provide coverage of pharmacy needs. • Provide patient counseling when required. • Supervises, coordinates, directs and/or participates in the preparation of chemotherapy and hazardous sterile intravenous admixtures, dispenses all pharmaceuticals and related supplies and equipment in accordance with federal, state and local laws and standards of practice which govern the profession of pharmacy. • Maintain adequate inventory levels of chemotherapy medications. • Prepares cytotoxic agents for administration employing hospital and department aseptic and protective procedures. • Ensures compliance with hazardous waste collection and disposal procedures. • Spends greater than or equal to 50% of the time manipulating, and/or compounding hazardous medications. • Responsible for chemotherapy and other high risk compounding, including desensitization compounds • Completes hazardous medication training and competencies. • Responsible for the accurate and safe compounding of medication admixtures in an environment set forth by USP 797 chapter and organization policies and procedures • Handle hazardous drugs according to USP 800 and NIOSH recommendations • Maintaining a clean room for sterile compounding, performing routine quality assurance monitoring, complying with regulatory and institutional guidelines for the disposal of hazardous drug waste, and perform other miscellaneous job related tasks/activities as assigned by the supervising pharmacist. • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy. • Process all orders in a timely fashion. • Receives, reviews and verifies all new patient orders. • Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding medications and their appropriate use. • Maintains pharmacy records in accordance with applicable laws and regulations. • Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars and conferences. • Participates in orientation/training programs for new personnel and current staff. Participates in the educational activities of pharmacy technicians and serves as a preceptor exposing them to all aspects of pharmacy practice. • Accurately administers all procedures involved with the procurement, inventory control, record maintenance. • Monitors drug therapy and communicates relevant findings to other health care professionals who are also responsible for patient care. • Participates in departmental Quality Assurance activities. • Attends department meetings as scheduled. • Performs other tasks or special projects as assigned by management. • Experience and strong knowledge base in intravenous admixture services, intravenous drug therapy and chemotherapy is required. • Must be able to work effectively with others to provide safe and effective drug therapy. • Ability to work in a fast paced environment. • Excellent problem-solving skills and the ability to work independently and quickly in responding to and resolving a wide range of problems. • Flexibility with work schedule. • Participation and membership in professional societies and organizations is desirable. • Excellent verbal and communication skills. • Dependable in attendance and job performance. • Good organizational and time-management skills. • Team player with a positive attitude and work ethic. • Oversee 340B program Position Requirments: • Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. • Doctorate of Pharmacy required. • Pharmacy PGY1 residency required plus 2-3 years of practice of oncology experience required.
