Responsibilities
Community Engagement/Management Professional Role Overview
Primary responsibilities include:
- Supporting the implementation of the Health Equity, Population Health, and Member Education Plans for the Louisiana Medicaid Market by working collaboratively with the Health Equity and Population Health team and Market team.
- Traveling throughout the region to provide education about the organization to promote health equity and community engagement. Resides in the same territory they are assigned to work in; exceptions to this may be considered, due to business need.
- Establishing relationships and actively engaging with communities and community-based entities to inform and address local health equity issues (Local, Regional).
- Supporting core strategies that include authentic community engagement, working in priority communities, using data to inform practices, and seeking feedback from Member, Provider, and Community Advisory Groups to inform the development and delivery of programs and services that eliminate health disparities.
- Working with the Health Equity and Population Health team to promote cultural humility at all levels within the organization and among its providers
- Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Required Qualifications
- Associates Degree in health, social services, communications, or related field or equivalent work experience of 2 years in related role
- Minimum of two (2) years of experience in community outreach, or related field of equivalent work experience
- Must reside in the state of Louisiana
- Ability to communicate effectively to senior leadership, team members, and with the community
- Experience working with cross-functional teams and translating requirements between business, project management and community projects or programs
- Intermediate to advanced computer skills and experience with Microsoft Word, Excel and Outlook
- Must be committed to improving the health of Louisianans and reducing health disparities
- Humana and its subsidiaries require vaccinated associates who work outside of their home to submit proof of vaccination, including COVID-19 boosters. Associates who remain unvaccinated must either undergo weekly negative COVID testing OR wear a mask at all times while in a Humana facility or while working in the field.
Work-At-Home Requirements
- Must have the ability to provide a high speed DSL or cable modem for a home office. Associates or contractors who live and work from home in the state of California will be provided payment for their internet expense.
- A minimum standard speed for optimal performance of 25x10 (25mpbs download x 10mpbs upload) is required.
- Satellite and Wireless Internet service is NOT allowed for this role.
- A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Travel
- This position will require a minimum of 75% travel (4-5 days a week) to community-based events and organizations as well as throughout the region you support in LA
- Must have a Valid driver's license with reliable transportation and the ability to travel within the state, as required
- This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
Preferred Qualifications
- Experience in Managed Care or Health Insurance
- Prior experience with Medicare & Medicaid recipients.
- Knowledge of community health and social service agencies and additional community resources.
- Community Health Certification
