Community Outreach Coordinator
Job Description
CORE (Community Organized Relief Effort) is a nonprofit organization whose mission is to build sustained, community-based response programs to natural disasters. In response to the COVID-19 pandemic, CORE GA provides free vaccinations and additional resources to disadvantaged communities and emerging hotspots around Georgia. The Health Outreach Coordinator will be responsible for outreach and education to organizations and individuals throughout communities served. Additionally, the Health Outreach Coordinator will manage the benefits navigation and incentives activities.
This is a hybrid position and will be in the field a minimum of 2-3 days a week. Some weeks may require 5 days in the field, and other weeks will be 2-3 days in the field and 2-3 days from home.
Responsibilities:
- Understand and effectively communicate CORE’s mission, vision, core values & strategic direction to external groups
- Conduct intake, follow up, refer, and assist clients to community resources and public assistance programs.
- Assist clients with screenings and applications to public assistance programs such as SNAP, WIC, TANF, Medicaid, etc.
- Develop trust and rapport with clients and refer to additional programs and resources as neededÂ
- Maintain and share community resources information with CORE GA program staff and clients
- Track and monitor program data and deliverables[MH1]
- Identify and develop relationships with resource organizations in designated Health District who can participate in/lead response activities
- Engage with formal and informal community leaders and influencers, including churches, local businesses, neighborhood associations, and other civic organizations
- Work closely with the Team Supervisor and Assistant Team Supervisor [MH2]Â [WF3]Â to provide a continuum of servicesÂ
- Adhere to all program guidelines, policies, protocols and workflows
- Lead health district strategic plan[MH4]Â [WF5]Â
- Report to Operations Manager and Team Supervisor for direction and communicate significant incidents
- Other duties as assigned
Qualifications:
- Minimum 1-2 years’ experience in social work or public health or other fields providing community resources and transformation
- Available Full-Time, 40 hours a week, including weekends (weekly schedule varies, 5 days/week)
- Live in, or near the local health district office
- Fluency in another language, particularly Spanish is preferred
- Access to reliable transportation to travel throughout assigned Health District
- Comfortable traveling up to 1-3 hours from work base in personal vehicle
- Comfortable working outdoors
- Demonstrated experience working with underserved/under resources communities
- Demonstrated experience in identifying and developing partnerships with organizations to meet needs of low-income clientsÂ
- Ability to empathize and establish supportive rapport with clients while maintaining professional boundariesÂ
- Excellent written and verbal communication skillsÂ
- Proficient in MS Office applications and referral platformsÂ
- Close attention to detail and a high level of accuracyÂ
- Bilingual in English and Spanish or another language highly preferred
- Flexibility to adapt to changing priorities and needs of the organization and adaptability in work style and work schedule, including ability to work for extended days and on weekends during critical points in the program
- Ability to lift up to 25 pounds
Experience:
- Demonstrated experience working with underserved/under resources communitiesÂ
- Demonstrated experience in identifying and developing partnerships with organizations to meet needs of low-income clients
Skills and Competencies:
- Excellent analytical skills, observation and analysis capacity
- Very good knowledge of G-Suite and Microsoft Office (including MS Word, Excel)Â
- Readiness to commit and adhere to the values, mission and vision of COREÂ
- Excellent writing, reporting and communication skillsÂ
- Proficient in MS Office applications and referral platforms
- Close attention to detail and a high level of accuracy
