Position Description

The Community Outreach Specialist reports to the Director of Outreach, CINQ Care Inc. This role is responsible for primary work in the community with specific target population’s in order to educate, enroll, and manage health home members. They work closely to meet and exceed our mission to provide a collaborative and comprehensive approach to healthcare, training and research. Primary Responsibilities

• The Community Outreach Specialist will have the following responsibilities: 

• Convey the purposes and services of a program to the user population and the impact that program or service would have 

• Assist in recruitment and retention of GBUAHN HH member 

• Assist care team to help patients develop health management plans and goals 

• Assist care team in coaching patients on problem solving, effective management of their chronic health conditions and self-care 

• Assist patient in understanding care plans and instructions 

• Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place 

• Liaison at sponsored community outreach functions 

• Work collaboratively and effectively within a team 

• Establish positive, supportive relationships with participants and provide feedback 

• Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible 

• Assist clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services 

• Assist CINQ members in conduction community strength and needs assessment as part of the care coordination process 

• Assist clients in translation and interpretation where appropriate 

• Motivate patients to be active and engaged in self-management and promote treatment adherence 

• Advocating and promoting health and wellness, utilizing health volt as tool for implementation 

• Advocate for harm reduction 

• Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program. 

• Identify and apply appropriate role definition and skilled boundaries

The Community Outreach Specialist shall have the following duties:

• Addresses all member concerns in a timely and efficient manner and reports any complaints to Care Coordinator for resolution. 

 Compliance: 

• Complies with quality assurance, OSHA, HIPAA, patient centered medical home focus, infection control, safety and other policies set forth. 

• Maintains competence through continuing education and in-service training 

• Attendance and being on time, ready to work your scheduled hours is an essential job requirement.

Qualifications

The Community Outreach Specialist should have the following qualifications: 

Education: 

• Associate degree required; B.S or B.A. preferred. 

• Prefer 3-years’ experience in a health and social services environment or education 

Experience: 

• Working background in health and social services preferred. 

• Verifiable good driving record and reliable transportation 

• Bilingual/bicultural (Spanish) encouraged to apply Technology: 

• Computer literacy, including proficiency in Microsoft Word and use of online reference and information resources required.

Pay: $22 hourly.
