Position Summary:
The Community Partnerships Manager with a focus on social determinants of health and health equity, will play a key role in developing and cultivating relationships within the community to address various social factors that influence health outcomes. The primary responsibility will be to develop and execute the organization's efforts in building impactful partnerships with community-based organizations and other key agencies. By leveraging your expertise, the Community Partnerships Manager will drive collaborative initiatives and implement programs aimed at improving community health and well-being.
The Community Partnerships Manager is expected to maintain strong knowledge of the community resources landscape and at-large community partners in addition to understanding community dynamics and needs, leveraging existing and accessible data. They will represent Equality Health at various community events and meetings and may work outside of regular business hours and require local and in-state travel.

Responsibilities:
- Develop and execute a comprehensive partnership strategy focused on quality metrics and addressing social determinants of health, aligning with organizational goals and objectives.
- Forge partnerships with local nonprofits, state and county agencies as well as community-based and faith-based coalitions. Collaborate with these stakeholders to develop programs that address health inequities and address social determinants of health.
- Lead the design and implementation of innovative programs and initiatives that focus on social determinants of health, ensuring they are evidence-based, culturally humble, and scalable to new markets.
- Utilize data and metrics to assess the effectiveness of partnership initiatives, identify areas for improvement, and make data-driven recommendations for enhancing outcomes and impact.
- Support partners in identifying and securing funding opportunities, to support mutual initiatives.
- Stay abreast of industry trends and maintain knowledge of current and developing public health, health equity, social determinants of health and healthcare reform practices and research and ground their work through such lenses.
- Work closely with Care Teams to identify opportunities to leverage clinical initiatives into partners' workflows and identify local community partners that meet member needs.
- Stay up to date with SDOH closed-loop referral system(s) and train new staff. Provide support with such tools to partner organizations as needed.
Health Equity
- Support the Director, Health Equity and Community Partnerships in the implementation, and evaluation of initiatives directly related to health equity and organizational health equity accreditation.
- Collaborate with cross-functional teams to integrate health equity principles and practices into programs, policies and operations throughout the organization.
- Monitor state and federal policy developments related to health equity.
Workflow Consultation
- Serve as a consultant to partners to support the success of the relationship. Expectations include:
- Analyze partners' workflows, identify opportunities for improvement, and suggest new workflows.
- Lead the execution of the new workflow, monitor for results and adjust or redesign as needed.
Database Management
- Maintain internal protocols to support partnership agreements with community resources and accurately report efforts through regular updates to CRM.
- Leverage databases (internal and external) to create usable and actionable reports for community solutions.
Required Knowledge, Education, & Experience:
- Bachelor's degree in Nonprofit Studies, Social Work, Public Health, Healthcare Administration, or a related field of study; or, and equivalent combination of education and/or experience.
- Minimum five (5) years of experience in a directly related position in a community setting or healthcare.
- Demonstrated knowledge and understanding of assessing and addressing social determinants of health.
- Demonstrated knowledge and understanding in the practice of community engagement and partnerships.
- Successful record of managing multiple projects with demonstrated ability to work independently and develop partnerships in rapidly changing environments.
- Proficiency with Microsoft Office applications and web-based technologies.
Highly Preferred Skills, Abilities, & Qualifications:
- Commitment to health equity, social justice, and a deep understanding of the structural factors that contribute to health inequities.
- Strong understanding of equity-centered approaches, cultural humility, and the ability to navigate complex systems and organizational structures.
- Experience leading and supervising teams.
- Familiarity with community needs assessments, program evaluation methodologies, and impact measurement frameworks.
- Deep understanding of the value of cultural humility and can work and collaborate with stakeholders from a variety of backgrounds.
- Bilingual; able to read, write and speak Spanish and English proficiently.
- Strategic mindset, critical thinker and innovative 'out-of-the-box' thinker.
- Excellent public speaking, facilitation, and presentation skills.
- Excellent problem-solving skills, including the ability to analyze problems, draw relevant conclusions and devise appropriate courses of action.
- Strong communication and interpersonal skills, with the ability to engage and collaborate with diverse stakeholders, build consensus, and influence change.
