CTI Registered Nurse (RN)
Job Description
This is an exceptional opportunity for an experienced and compassionate Registered Nurse to serve in a vital capacity on one of the Governor’s Office’s newly launched and innovative Critical Time Intervention (CTI) teams. As an integral member of this multidisciplinary team—comprised of a Director, Assistant Director, Licensed Clinicians, Care Managers, a Peer Specialist, and a Registered Nurse—the CTI RN will provide comprehensive, community-based care to vulnerable individuals during critical periods of transition, particularly following discharge from institutional settings. The RN will play a key role in addressing the physical and medical health needs of participants, many of whom face significant challenges such as housing instability, limited access to traditional healthcare services, and the cumulative effects of physical and social determinants of health. Utilizing the evidence-based CTI model, the nurse will deliver interventions aimed at restoring and supporting health and well-being, connecting individuals to primary and specialty care, and preventing avoidable hospitalizations. Additionally, the RN will contribute to the development of individualized care plans, provide essential nursing services, and ensure appropriate referrals for psychosocial support. This field-based role requires flexibility, including periodic on-call responsibilities and the ability to work non-traditional hours, as the team supports participants over a structured nine-month period to foster long-term stability, independence, and improved health outcomes.
Essential Position Functions:
• Conduct, interpret, and monitor comprehensive assessments of participants' physical, cognitive, and psychosocial health and well-being.
• Engage with participants beginning at the point of inpatient hospital admission or emergency department visit, participating in transition planning, including a thorough needs assessment for essential community-based supports to ensure stabilization post-discharge.
• Plan, implement, and evaluate care, interventions, and treatments based on evidence-based practices to address both fundamental and complex health needs.
• Reassess participants when changes in condition, needs, or deterioration are observed, including monitoring the effects of diet and physical activity.
• Establish and maintain active connections between participants and community-based resources that promote overall health and well-being.
• Provide physical and psychological support to participants within community settings and in their homes.
• Offer consultation and collaborate effectively with team members and external agencies, ensuring cultural sensitivity, confidentiality, and respect for participants’ rights.
• Develop and maintain effective working relationships with healthcare providers in both acute and non-acute settings to advocate for the best interests of participants.
• Promote community awareness regarding the needs of current and potential participants, as well as available resources to address their care needs.
• Participate in team and administrative meetings, contributing nursing expertise to the development and implementation of individualized treatment and service plans.
• Contribute to risk management and quality assurance processes related to health care delivery.
• Conduct medication reconciliation, provide education on prescribed medications, and monitor for and document therapeutic effects and potential side effects.
• Perform other related duties as assigned.
Minimum Education and Experience Requirements:License and current registration to practice as a registered nurse in New York State; prefer at least one- year post RN licensure clinical nursing experience working with individuals with behavioral health needs.
Required Knowledge, Skills, and Abilities:
• Possession of a license and current registration to practice as a Registered Nurse in New York State.

• Demonstrated knowledge of professional nursing concepts, principles, and practices.
• Understanding of biological science concepts and principles relevant to nursing practice.

• Ability to recognize abnormal signs and symptoms related to both physical and mental illness, as well as indicators of wellness.
• Knowledge of psychiatric and general health medications, including common dosages, therapeutic effects, and potential psychological, physical, and physiological side effects or adverse reactions.
• Understanding of emergency treatments and appropriate nursing interventions.
• Knowledge of the agency’s policies, procedures, and operational routines.
• Awareness of cultural, ethnic, and linguistic factors that impact care, and the ability to provide culturally competent services.
• Ability to identify significant changes in a client’s condition that require immediate medical attention.
• Skilled in developing, evaluating, implementing, and modifying nursing care plans tailored to individual client needs.
• Ability to collaborate effectively and work cooperatively with interdisciplinary team members and external partners.
• Proficiency in preparing accurate, clear, and timely reports.
