Director of Social Services

Our Director of Social Services will advocate for our residents, families, staff and provide overall leadership and management. This is a Full-Time, exempt, benefit-eligible position working Monday through Friday.
Director of Social Services Responsibilities:

• Provide guidance and direction to the social services department through policy and procedure creation, along with monitoring to ensure the Cassia mission is being realized.
• Coordinate caseloads, complete social history, care plans, MDS, and discharge plans for new residents.
• Serve as backup to admissions director.
• Ensure campus is at maximum potential occupancy.
• Participate in resident care conferences involving the planning and implementation for medically related social services, mental health services, and emotional support services.
• Oversee and complete referrals for resident mental health services and coordinate consultative services with providers.
• Complete social services admission evaluation, as well as discharge, care plans for residents.
• Maintain necessary documentation in resident's medical records.
• Perform other duties as assigned.

Director of Social Services skills and requirements:

• Must be a Licensed Social Worker (LSW) in Minnesota.
• Prior experience in marketing or sales position preferred, but not required.
• Prior experience in the admission process, discharge planning, or management preferred.
• Exceptional time management and communications skills.
• Strong knowledge of local, state, and federal guidelines related to long-term care and resident rights.
