Job Description

What You’ll be Doing
The Case Manager in our Aging Well Program will work collaboratively with the Office of Healthy Aging (OHA) to provide case management services to clients who are part of the Office of Healthy Aging and Department of Human Services (DHS). The Case Manager provides in-home visits, monthly calls, and assists with care coordination to ensure client needs are addressed. 
Your Main Priorities
• Manage cases assigned by intake coordinator.
• Conduct initial and ongoing in-home assessment to determine care needs.
• Create care plans and recommend services to address individual needs.
• Follow-up with clients (frequency) after initial plan implemented to reassess needs.
• Collaborate with family/caregivers, service providers, and local authorities, as necessary.
• Maintain on-call hours for crises, as assigned.
• Complete all paperwork promptly and maintain accurate client records.
• Advocate for services to meet elder’s needs. 
• Other duties as assigned.
Qualifications
• Bachelor’s degree in social work or related field required.
• 1+ year experience in case management role or similar preferred
• Experience working with the elderly is a plus.
• Ability to communicate and effectively interact with a diverse population. 
• Bilingual in Spanish strongly preferred.
• Strong organizational and time-management skills.
• Basic computer skills required; knowledge of Microsoft Office 365.
• Must have a valid driver’s license and auto insurance.
• Must be able to authorize Child & Family to complete Motor Vehicle Record check.
• The ideal candidate will be required to meet all pre-employment requirements such as background checks and medical clearances.
Travel Requirements
Site location: Middletown, RI and Providence, RI
Local travel required.
