Applewood Centers is a premier provider of community-based behavioral health services in Northeast Ohio. Applewood is seeking dynamic and dedicated individuals who want to grow their behavioral health careers and become part of an amazing team of professionals serving others. We are currently hiring a Clinical Therapist to serve youth (age 2-20) and families who call the crisis hotline with a family identified emergency.

Position Summary:

The Case Manager provides intensive home and community based skill building and case management services to seriously emotionally and behaviorally troubled (SED) youth and their families. Maintains a caseload of youth and families. Caseload size may vary based upon the needs of assigned families. Engages clients and families as partners in the care planning process and operates from a strengths perspective.

Responsibilities Include:

  • Interviews clients to obtain information pertinent to establishing and maintaining effective client records and service plans (i.e., PDE, ITP). Gathers information in a timely and systemic manner that will enable ongoing diagnosis, assessment, and treatment planning.
  • Demonstrates the ability to develop and maintain strengths-focused, professional alignment and engagement with families.ย 
  • Develop focused, measurable, and achievable service plans that are consistent with the family's identified needs and cultural assessment.
  • Assists and advocates on behalf of clients and families to secure needed resources, services, and supports.
  • Collaborates with families and clients in the development of crisis plans and provides crisis assistance and coaching as needed.
  • Assists clients and families with the development of skills that will enhance and improve functioning in areas that include, but are not limited to, interpersonal relationships, anger management, and coping.ย 

Qualifications:

  • Minimum: bachelor's degree in social work; graduate degree preferredย in social work or counseling.ย 
  • Professional licensure in the State of Ohio in social work or counseling (BSW/LSW or LPC)
  • A valid driver license, personal transportation, and auto insurance.
