We pride ourselves on valuing our patients and staff, and would love to have you join the team! **Company vehicle provided** The RN case manager is responsible for admission and ongoing evaluation of patients in-home health care, ensuring that the care provided by the interdisciplinary team is patient-centered, safe, timely, cost-effective, and performed with excellence. Essential Responsibilities: - Perform initial in-home assessment of patient to determine healthcare needs - Initiate, develop, and execute a plan of care (485) - Administer medications and treatments as prescribed by physician - Communicate with physician regarding patient’s needs - Counsel patient/caregiver on how to best meet patient’s needs - Participate in regular case conference and in-services Qualifications: - Current RN license in the state of Texas - Minimum of 1 year of experience in home health setting with OASIS experience required - Understanding of PDGM requirements - Valid state driver’s license and automobile liability insurance - Demonstrate knowledge and skill in current nursing practice - Ability to make independent nursing judgments with minimal supervision - Must demonstrate sound and proven skills in performing clinical procedures, expert level in verbal, written, and electronic health records, and superior time management and organizational skills - Experience with healthcare software,Kinnser/WellSky required Job Type: Full-time Pay: $82,000.00 - $84,000.00 per year Benefits: • 401(k) • 401(k) matching • Dental insurance • Health insurance • Paid time off • Vision insurance Medical Specialty: • Home Health Application Question(s): • Describe your experience with OASIS.
