Position Overview:

As a Geriatrician at VHC Health, you will play a vital role in providing specialized medical care to senior residents. You will collaborate with a multidisciplinary team of healthcare professionals to ensure the well-being of residents while handling a range of services including on-site primary care, chronic and acute care management, care coordination, and transitional care. The Geriatrician will diagnose and treat a broad range of medical conditions specific to the geriatric population, provide comprehensive care plans and provide personalized treatment plan which promote older adults to age in place.

Key Responsibilities:

• Conduct thorough medical assessments, diagnoses, and treatment plans for older adults
• Focus on management of polypharmacy
• Collaborate with the interdisciplinary team and specialists to develop and implement comprehensive care plans
• Provide guidance and education to staff, residents, and their families on geriatric care
• Oversee quality assurance performance improvements initiatives and policies specific to the care of older adults
• Participate in care conferences and interdisciplinary meetings
• Option to start/grow an ambulatory practice for non-facility based geriatrics patients within the community, co-located with our primary care providers

Schedule:

• Regular business hours of operations: Monday – Friday
• Flexible schedule
• Flexibility for administrative time built into the schedule
• Collaborate with a Team of Geriatricians and other Specialists onsite

Requirements:

• Medical degree from an accredited institution
• Board certification or eligibility in Family Medicine or Internal Medicine; Geriatric Medicine Fellowship preferred
• Current, unrestricted medical license in Virginia
• Strong understanding and experience in geriatric medicine
• Excellent communication skills and ability to work collaboratively in a team environment
• Compassionate and person-centered approach to care
