Job Description:
Description: The HIM Associate II is responsible for reviewing medical records to assure all documentation is required is present in the medical record, processing all transcribed reports in the department, trains new providers on the dictation system.
Typical Duties:
- Monitors assigned workques in the EMR system.
- Reviews inpatient, day surgery, observation, emergency room records for completeness and assigns the appropriate deficiencies to medical staff as needed.
- Performs data integrity functions to include: marking/flagging medical record numbers for merging, merging medical record numbers, and patient overlays within the EMR.
- Performs all duties as it relates to reporting births and deaths to the Texas Department of Vital Statistics to include: interviewing new parents and completing the birth certificate information in the Texas Electronic Registry (TER), assisting non-married parents with the state acknowledgement of paternity documentation, assist funeral homes, medical examiner's offices, and other state agencies regarding information pertaining to a death registration, working as a liaison between funeral homes and JPS Medical Staff regarding death certificate completion.
- Processes requests for protected health information (PHI) within the timeframes established by departmental policy and procedure and according to federal, state, and regulatory guidelines.
- Prepping, Scanning, and Indexing; primarily responsible for collecting and reconciling discharges from clinical areas and for prepping-scanning-indexing medical records of discharged patients to be uploaded into the HIM Electronic Document Management System (EDMS) ChartMaxx.
- Performs quality audits and maintains statistics as assigned to ensure the accuracy of services provided.
- Performs other duties as assigned within the scope of knowledge.

Qualifications:
- Required Education and Experience:
- High School Diploma, GED, or equivalent.
- 1 plus year of previous experience working for a healthcare provider such as a hospital, clinic or physician office.
- Successful completion of medical terminology course.
- Team members performing the birth registrar function are required to have a State of Texas Certification for the Acknowledgement of Paternity program within six months of hire issued through the Office of the Attorney General.
