Summary:

The HIM Document Integrity Analyst II will evaluate patient records for inclusion, accuracy, and completeness of required documentation as defined by The Joint Commission, state, and federal regulations. The HIM Document Integrity Analyst II assigns, validates, and reassigns chart deficiencies based on CCMC Medical Staff Rules and Regulations. The HIM Document Integrity Analyst II assists physicians with reconciliation of incomplete medical records. The HIM Document Integrity Analyst II performs ongoing medical record reviews to identify and report documentation issues.

Education & Experience:

• Must have an Associate's degree in Health Information Management OR must have a High School Diploma or equivalent with four (4) years of hospital-centered office experience related to analysis and/or auditing of the medical record.
• Requires a broad knowledge of medical terminology, record management concepts, data integrity of patient related information, and HIM office applications.
• Must be proficient in Microsoft Office software.
• Must be detail- oriented, analytical, have strong organizational, writing/interpersonal communication skills, and the ability to maintain confidentiality.
• Must work well independently with minimal supervision and have the ability to solve problems appropriately using job knowledge and current policies and procedures.
• Epic experience highly desired.
• This position has the potential to be a remote/telecommute position.

Certification/Licensure:

• Registered Health Information Technician (RHIT) preferred.
