A Brief Overview
Under general supervision, plans, organizes and controls the activities related to requests for medical information from multiple external requesters. Monitors the clerical personnel utilized by outside copy services relative to quality of work and timely completion of requests.

What you will do
- General office:
- Answers incoming phone calls and responds to in-house requests for medical information.
- Medical record request processing:
- Receives all requests for medical information from attorneys, physicians, hospitals, clinics, patients, etc.
- Logs all incoming requests and distributes as appropriate within 3 days of receipt.
- Responsible for completing all requests within standard timeframes as follows:
- Patient Requests - 7 business days.
- Billing Requests - 7 business days.
- Subpoenas - Within required time frame according to
- request.
- Follow up care - Faxed or mailed immediately upon
- request with signed release.
- Screens requests to ensure that each one is complete and valid.
- Responsible for fulfilling requirements as specified in subpoena and court orders.
- Processes subpoenas within the specific time allowed and by appearing and testifying in attorney offices, Federal, State, and County courts when required.
- Monitors the currently contracted copy service personnel relative to quality and timeliness of work completed.
- Communicates with service staff to ensure the 1:1 ratio is used effectively.
- Receives logs and distributes all monetary payment for release of information requests:
- Responsible for completing a weekly deposit register and delivering it to the Patient Financial Services department.
- Assists the HIM department management team with the development and revision of hospital policies and letters of correspondence regarding confidentiality and release of patient information as needed.
- Communicates and notifies the Risk Manager, Administration and Physicians of potential litigation as identified by information requests and patient complaints.
- Attends at least one legal seminar update annually. Reports back all legal update material within one week.
- Trained to pull charts according to the surgery schedule as needed.
- Maintains, researches and corrects all information for incorrect or double medical record numbers in a timely manner.
- Obtains, reviews, and analyzes all patient medical record discharge types for completeness:
- All medical records are retrieved, assembled, and analyzed within 24 hours of discharge.
- Identifies deficiencies by physician for each discharged medical record.
- Performs data entry of chart deficiencies as appropriate in the hospital computer system.
- Responsible for keeping within productivity standards set by the management team for analysis and assembly of patient discharges.
- Records are accurately filed in the appropriate file area immediately upon completion (initial/recheck).
- Cross trained to be part of the coverage in either HIM office if requested by the management team:
- Chart management procedures specific to the Livermore campus.
- Chart management procedures specific to the Pleasanton campus.
- Monitors the dictation and transcription systems for completed reports:
- Responsible for the distribution, filing and tracking of all dictated reports.
- Updates incomplete charts as needed when new transcribed reports are filed.
- Responsible for the importing of transcribed documents in the absence of transcription coordinators.
- Participates in ongoing department projects:
- Takes an active role in the annual purge of records to/from storage.
- Participates in the review of specific file areas for accuracy.
- Works effectively and harmoniously with others.
- Accepts responsibility for department work environment:
- Maintains a neat, orderly, and professional workspace.
- Responsible for participating in the copying of records for billing purposes etc.
- Provides exceptional customer service at all times.
- Works independently and exercises initiative, judgment, and tact.
- Performs other job duties as assigned by the management team.
- Responsible for participating in weekend and holiday coverage as outlined on the monthly schedule
Education Qualifications
- High School Diploma or GED
Required Knowledge, Skills and Abilities
- Basic computer skills including knowledge of how to access computerized data and perform data entry utilizing 10 key.
- Medical office, acute care hospital or other health care setting experience preferred.
- Knowledge of current legislation regarding release of information to ensure familiarity of various and ever changing requirements.
- Well organized and able to work under time constraints.
- Tack and ability to work with various professional disciplines: physicians, attorneys, etc. Works with patients requesting access to records.
Licenses and Certifications
- ART - Medical Records Coder preferred

Base Pay Scale: Generally starting at $27.23 - $33.42 per hour
The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
